WE ARE PIQUA! Welcome to Piqua City Schools! We are excited to include your family as part of our school community!
Piqua City Schools uses a centralized enrollment process. Enrollment must be completed by a parent or guardian who resides within the Piqua City School District. The online enrollment can be completed on a laptop, desktop, tablet, or smartphone.
Students entering Kindergarten MUST be 5 years old by the first day of school for the school year for which they are being registered.

Piqua City Schools uses a centralized enrollment process. Enrollment must be completed by a parent or guardian who resides within the Piqua City School District. The online enrollment process can be completed on a laptop, desktop, or another device. Computers are available at the Piqua Board of Education at 215 Looney Road.
To begin, you will need an active email account to create a NEW ACCOUNT which allows you to begin the registration process, save your information, and return at a later date for completion (if necessary). After you have submitted your enrollment application, please bring ALL required documentation to complete the enrollment process for your student to the Board of Education office by appointment only from 9:00 AM - 3:30 PM.
ENROLLING NEW STUDENTS IN PIQUA CITY SCHOOLS
Please complete the following steps for student registration:

Go to the following link to register for a parent account. If you already have a parent account for Piqua City School’s FinalForms, please go Final Forms and select login

Locate the parent icon and click NEW ACCOUNT, then follow the steps to create your account. If you already have a parent account for Piqua City School’s FinalForms, please select login.

Type YOUR NAME, DATE OF BIRTH, AND EMAIL. Next, click REGISTER.
NOTE: You will receive an email within 2 minutes prompting you to confirm your registration. If you do not receive an email, then check your spam folder. If you still cannot locate the FinalForms email, then contact the Board of Education at 937-773-4321 to resolve the issue.
- Check your email for an ACCOUNT CONFIRMATION EMAIL from FinalForms. Once received and opened, click CONFIRM YOUR ACCOUNT in the email text.

Create your new FinalForms password. Next, click CONFIRM ACCOUNT.

Click REGISTER STUDENT for your new student at Piqua City Schools.

Complete the online enrollment application.

After you have submitted your enrollment application through FinalForms, please contact the Board of Education at 937-773-4321 to schedule a time to drop off ALL required documents to complete the enrollment process. (See the required documentation below)
Please note: Before your child can begin to attend school, the enrollment application and ALL required documentation must be submitted!

Proof of Residency (gas, electric, or landline phone bill). Please note that you must be currently residing in the Piqua City School district to enroll your student. All new Piqua City School District students must provide the proper documentation to establish proof of residency. The name and address on these documents must match the name and address of the custodial parent or legal guardian of the student(s) being registered. If you are living in the Piqua City School District and have not yet received an electric, gas, landline phone bill, or MONTHLY utility bill, you can provide your signed lease agreement or closing paperwork.

Piqua City Schools’ Kindergarten Registration for the 2026-2027 school year opens March 2, 2026, and we are looking forward to welcoming our newest learners to Piqua City Schools!
Important Kindergarten Age Requirement Update!
Children living within the Piqua City School District, who will be 5 years old on or before September 8, 2026, are eligible to register. This age requirement follows Ohio House Bill 114, which standardized kindergarten enrollment statewide. Under this new law, public schools must admit any child who turns five by the first day of instruction for the school year. If your child will be five years old on or before September 8, 2026, they can be part of the Piqua Indians Class of 2039!
Piqua City Schools will offer registration for kindergarten students living within the Piqua City School District starting March 2, 2026, every Monday, Wednesday, and Friday from 9:00 AM - 3:30 PM at the Board of Education office by appointment only (your child does not need to attend). Kindergarten registration must be completed by a parent or legal guardian who resides within the Piqua City School District. To enter kindergarten, students must turn five years of age on or before September 8, 2026.
Please Note: If your child currently attends preschool, you WILL NEED to register your child for kindergarten.
To begin the registration process, please follow the steps below:
ENROLLMENT PROCESS

Collect Required Documents for Registration. Please refer to the Student Enrollment Guidlines for further details on the required documents.

Click here for the FinalForms account and select the 2026-27 school year to begin the online enrollment process by logging into your FinalForms account or by creating a parent account if you do not already have one. Please note that new accounts need to have a current working email address to complete the parent registration process. New will need to confirm your account through the email sent by FinalForms.

Once logged in to FinalForms, you will need to switch the registration date to the 2026-27 school year. Then select Add Student and complete the student information. After the student information has been added, complete all of the enrollment forms on FinalForms.

Once you have completed all information within FinalForms and collected all of the required documents above, please call the Board of Education office at 937-773-4321 to schedule an appointment. The student does not need to attend this appointment.

Attend your enrollment appointment at the Board of Education, located at 215 Looney Road, Piqua. Please bring the required documentation to this appointment (copies will be made of your documents and the originals will be returned to you)
Walk-In Registration
For those who wish to complete registration on-site (Piqua Board of Education office at 215 Looney Road), call 937-773-4321 for an appointment (hours are 9:00 AM - 3:30 PM) and bring the required documents for registration from the items listed above.

Back-to-school and athletic participation forms for your students for the 2026-2027 school year are now available on FinalForms.
You are required to review and sign your forms once per year and provide updates to any changes in your child's information, including health, address changes, parent contact information, or when additional forms are added. Forms may be updated at any time throughout the school year. If you need to update your child's bus drop-off or pick-up information after you have completed the Transportation Information section on FinalForms, you may call the transportation office at 937-773-1560 and complete the Google Forms link in the Transportation Information section of FinalForms.
